The University of Arkansas is committed to effectively managing the ever-increasing cost of health care.
A key to controlling costs is to ensure that our benefit plans are providing coverage only to eligible employees and their eligible spouses and other dependents. To ensure that all of our health care dollars are being spent according to plan eligibility rules, the university will be conducting a spousal verification audit.
The verification project will begin this month (January 2024). If you are currently enrolled in employee-plus-spouse or employee-plus-family coverage, you will receive an email in late January from Part D Advisors (PDA) to your work email address to begin the process. In addition, a form will be mailed to your residence asking that you provide documentation verifying your spouse’s eligibility for coverage. You have until February 20, 2024, to provide the required eligibility documentation.
What documentation will be required to verify my spouse’s eligibility?
You will be required to provide a copy of either your 2023 or your 2022 filed federal income tax return Form 1040. Be sure to blacken out all social security numbers (SSNs) and financial information.
You must submit required documentation either via USPS mail or through the secure website. Your response is mandatory.
