UMR Live Well Reward$ Wellness Program

For employees who are enrolled in the UMR medical plans, the university offers the opportunity to qualify for lower out-of-pocket limits and to earn wellness rewards by opting in to UMR’s Live Well Reward$ wellness program.

Medical Plan Savings

If you are enrolled or plan to enroll in the Classic Plan or the Premier Plan and you opt in to the Live Well Reward$ program, you’ll automatically receive the lower medical out-of-pocket maximum:

  • Classic Plan: You’ll save $1,250 (individual coverage) or $2,500 (family coverage) on your medical plan’s annual out-of-pocket maximum.
  • Premier Plan: You’ll save $500 (individual coverage) or $1,000 (family coverage) on your medical plan’s out-of-pocket maximum.

For the Health Savings Plan, there is no reduction in out-of-pocket maximums; however, you can still earn wellness points.

Wellness Program Points

Through this online wellness program, you can earn up to 500 total points, redeemable for up to $500 a year (1 point = $1) by completing certain wellness activities, screenings and assessments, including:

  • Annual Wellness Visit (50 points)
  • Breast Cancer Screening (50 points)
  • Cervical Cancer Screening (50 points)
  • Colorectal Cancer Screening (50 points)
  • Prostate Cancer Screening (50 points)
  • Cholesterol Screening (50 points)
  • Diabetes Screening (50 points)
  • Flu Vaccine (50 points)
  • Action Plans (50 points each, up to 250 points maximum)
  • Ongoing Condition CARE Coaching Engagement (must complete three coaching calls to be considered engaged) (100 points)

You can earn up to 200 points through preventive screenings and exams, and up to 300 points by participating in other activities.

Note: Wellness rewards are for university employees only. Dependents can participate in the wellness activities but cannot receive incentives.

Redeeming Wellness Program Points

You can redeem points for your choice of digital or physical gift cards, popular merchandise, travel vouchers and more. Visit umr.com to review details on the wellness activities and incentives.

All points are forfeited upon termination of employment, when employees end their university health plan enrollment or when employees are no longer enrolled in the wellness program.

Use it or lose it

Reward points do not roll over to the next year. You must use your accrued points for the 2025-2026 program year by November 30, 2026. Unused points will be forfeited. Visit umr.com to convert your points to gift cards.

Opting Into the Wellness Program

The 2025/2026 wellness program runs from October 1, 2025 – September 30, 2026. You must opt in through umr.com from October 1 to November 3, 2025 to be eligible; otherwise, you will have to wait until fall 2026. You must opt-in each year to participate; your enrollment does not carry over automatically.

Points accrued on and after October 1, 2025, are credited toward the 2025/2026 program year.

New hires on and after October 1, 2025, are not eligible to opt into the wellness program for the 2025/2026 program year. The next opportunity to opt in will be during fall 2026.

Real Appeal Weight Loss Program

Real Appeal is an online weight loss program. It’s available at no additional cost to university employees who are enrolled in a university medical plan and have a BMI of 23 or higher (subject to eligibility). Real Appeal is designed to help you lose weight and live a healthier life.

Here’s what you’ll get with Real Appeal:

  • Up to a year of support from a Transformation Coach. This is the person who guides you through the program and develops a simple, customized plan that fits your needs, preferences and goals.
  • Unlimited access to Real Appeal’s digital content library. The library includes workout videos; tools to help you track food, activity and weight; weekly analysis and feedback; recipes and more.
  • Access to weekly online coaching sessions with your Transformation Coach and a group of fellow members. Session topics include emotional eating, mindset, lack of motivation, and adding more behavior change and psychology tips.
  • A success kit. It’s full of weight management tools, including a perfect-portion plate, a weight scale and access to a digital fitness subscription.

HealthSafe ID needed starting June 1

Starting June 1, 2025, you will be required to use a HealthSafe ID to create your user account and to log in to the Real Appeal portal.

HealthSafe ID is an identity management application that includes multi-factor authentication for additional security.

  • Digital: $10 a month for live or on-demand online fitness classes and routines created just for you, no matter your experience level
  • Classic: $29 a month for access to 11,000+ gym locations and grocery delivery services
  • Standard: $64 a month for access to 12,000+ gym/fitness locations and grocery delivery services
  • Premium: $99 a month for access to 14,000+ gym/fitness locations and grocery delivery services
  • Elite: $144 a month for access to 16,000+ gym/fitness locations and grocery delivery services

Note: A one-time enrollment fee will apply. After signing up, you will be charged on the first of every month. Your enrolled family members will not pay an enrollment fee; plus, they receive a 10% discount and can choose any membership level (it doesn’t have to match your selection).

To register, select Sign In and add your HealthSafe ID (HSID) username and password to create an account. If you do not have a HSID, create one by selecting Get Started and inputting your first and last name, date of birth and identification type (SSN or UMR group number ID). Once enrolled, use the member code in the top-left-hand corner of the screen to verify your eligibility at fitness locations and register for new online services.
