You have the option to enroll in three employee-pay-all supplemental health insurance plans through UnitedHealthcare:
Critical Illness: Financial protection if you’re diagnosed with a serious illness
Hospital Indemnity Protection: Financial protection for hospital stays and associated treatment costs
Accident Protection: Financial protection for unexpected injuries or accidents
You can enroll in one, two or all three plans. Plus, each plan offers a $50 annual wellness benefit that you can use to help offset the cost of this valuable coverage.
If a serious illness or injury resulting in medical expenses would be a financial burden, consider whether one (or more) of these plans could provide some peace of mind.
Enrolling in Coverage
Enroll in the UnitedHealthcare plans through Workday.
You have 31 days from your first day of employment to enroll. If you do not enroll during this initial 31-day time period, you can only enroll:
During open enrollment (generally, in the fall of each year), or
Within 31 days of a qualifying life event.
Cost of Coverage
What you’ll pay depends on your plan option and the dependents you choose to cover. To review plan limitations, read each plan’s Summary of Benefits, listed on this webpage under “Important Documents.”
If you have a covered illness, injury or hospitalization, submit a claim to UnitedHealthcare, and the plan will pay a tax-free benefit directly to you.
Cash benefits are in addition to any benefits paid by your university medical coverage. There is no offset or coordination of benefits between these supplemental plans and your university medical coverage.
There’s no deductible and you can use the money however you wish. For example, you can use the cash benefits to reimburse yourself for:
Copays and deductibles
Travel expenses to treatment centers
Other expenses not covered by insurance
File a Claim
Call to submit a claim: (866) 556-8298, 8:00 AM to 8:00 PM (ET), Monday through Friday.
Or, submit a claim using a paper form. Complete the form that is applicable for your coverage and the type of claim you plan to submit:
Hospital indemnity insurance pays a lump-sum cash benefit directly to you if you have a covered hospitalization. Covered hospital expenses include hospital admission, hospital confinement, and intensive care unit (ICU) confinement.
You can choose from three plans:
1500 plan (pays $1,500 for an initial hospital admission)
1000 plan (pays $1,000 for an initial hospital admission)
500 plan (pays $500 for an initial hospital admission)
Accident insurance pays a lump-sum cash benefit directly to you if you have a covered injury. The plan covers more than 80 injuries and services, including:
Emergency room and urgent care
Hospital admissions and stays
Medical appliances (e.g., crutches, wheelchairs, walkers)
You can choose from three options:
Option 1: $20,000 lifetime benefit
Option 2: $30,000 lifetime benefit
Option 3: $50,000 lifetime benefit
You can earn $50 per year for each plan you elect (critical illness, hospital indemnity and/or accident) just for staying on top of your health. Use the money however you’d like — no restrictions are imposed by the university or by UHC.
Your covered spouse can also earn a $50 wellness benefit by completing an exam or screening.
If you enroll in more than one plan, you must complete a separate screening/test for each plan in order to earn multiple wellness credits.
For new 2023 enrollments, screenings must be completed during the 2023 calendar year.
The benefit will only be paid one time per calendar year and per plan, regardless of the test results or number of tests performed. The benefit is paid in addition to any other payments you and your covered spouse receive under the policy.
Wellness benefit details are listed by plan on this webpage under “Important Documents.”
